The doctor and/or staff are away from the office periodically.  Please leave us a message and we will return your call as soon as possible.

Before your initial visit we will mail your paperwork to you so that you have the opportunity to fill it out before your visit with us.  Please arrive 15 minutes early to complete the new patient process.  

We kindly ask that you give us two business days notice to cancel or reschedule your appointment.  Short notice or no show may result in a fee.
